OCR description
The covered entity (CE), Harris Eye Care, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 6,267 individuals. The PHI included names, addresses, dates of birth, drivers’ licenses and social security numbers, claims and financial information, diagnoses/conditions, lab results, and medications. The CE notified HHS, the affected individuals, and the media. The CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance to the CE.
