OCR description
The covered entity (CE), Allison Transmission Health Care Program for Hourly Employees / Wellness Program for Salaried Employees, reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 5,330 individuals. The PHI involved included names, dates of birth, email addresses, and Social Security numbers. The CE notified HHS and the media; the BA notified affected individuals and offered complimentary credit monitoring services. The BA implemented additional technical safeguards to better protect PHI.
