OCR description
The covered entity (CE), VNS Health Health Plans, reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 103,775 individuals. The PHI involved included names, addresses, Social Security numbers, phone numbers, email addresses, The CE notified HHS, the affected individuals, the media, and provided substitute notice.
