OCR description
The covered entity (CE), Nice Pak Products Health and Welfare Benefits Plan reported that it experienced a hacking attack that compromised the protected health information (PHI) of 8,487 individuals. The PHI involved included names, addresses, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards.
