OCR description
The business associate (BA), SUNY at Buffalo School of Dental Medicine, reported that a software vulnerability affected the protected health information (PHI) of 756 individuals. The PHI involved included names, addresses, diagnoses, and financial information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards.
