OCR description
The business associate (BA), Aspen Dental Management, Inc., reported that it experienced a breach that affected the protected health information (PHI) of 62,183 individuals. The PHI involved included demographic and clinical information. The BA notified HHS and the affected individuals. In response to the breach, the BA provided complimentary credit monitoring and implemented additional administrative, technical, and security safeguards.
