OCR description
The covered entity (CE), Advanced Reproductive Health Center dba Chicago IVF, reported that its business associate (BA) experienced a cyberattack that exposed the protected health information (PHI) of 2,603 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, financial information, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE and BA offered complimentary credit monitoring services to affected individuals and the BA implemented additional administrative and technical safeguards to better protect PHI.
