OCR description
The covered entity (CE), Milan Eye Center, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 67,336 individuals. The PHI involved included names, Social Security numbers, dates of birth, phone numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the CE and offered free credit monitoring services.
