OCR description
Detroit Central City Community Mental Health, the covered entity (CE), reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 1,412 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
