OCR description
The covered entity (CE), Associates in Pediatric Dentistry, reported that several employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 9,556 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial information, and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI and retrained its workforce members on email security precautions.
