OCR description
The covered entity (CE), Indiana University Health, reported that a vendor of its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,191 individuals. The PHI involved included names, Social Security numbers, addresses, birthdates, claims information, and other treatment information. The CE and BA notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE and BA strengthened its administrative and technical safeguards and provided complimentary credit monitoring services.
