OCR description
The covered entity (CE), MedMinder Systems, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 12,146 individuals. The PHI involved included dates of birth and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative, technical, and security safeguards. The CE retrained its staff on the requirement to protect and secure PHI.
