OCR description
The covered entity (CE), Ryders Health Management, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 7,252 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and provided media notice. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
