Oak Valley Hospital District

Oak Valley Hospital District, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 284,629 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, diagnoses, lab results, medications, and claims information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach and OCR’s investigation, the CE revised its policies and procedures, retrained its workforce members, and implemented additional technical safeguards.
