OCR description
Nuance Communications, the business associate (BA), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,225,054 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, medications, and Social Security numbers. The BA notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the BA strengthened its administrative, technical, and security safeguards to better protect PHI.
