OCR description
The business associate (BA), Elevate Patient Financial Solutions, reported that an employee mailed out documents in which the protected health information (PHI) of 5,364 individuals was visible through the window of the envelope. The PHI involved included names, addresses, and Social Security numbers. The BA notified HHS and the affected individuals. In its mitigation efforts, the BA strengthened its administrative and technical safeguards to better protect sensitive data.
