OCR description
The business associate (BA), Blue Cross Blue Shield of Texas, reported that an employee inadvertently mailed the protected health information (PHI) of 3,708 individuals to the wrong addresses. The PHI involved included names and health insurance information. The BA notified HHS, affected individuals, and the media In response to the breach, the BA implemented additional administrative safeguards to better protect PHI.
