OCR description
The covered entity (CE), Tangoe, Inc., reported that it was the victim of a ransomware incident that affected the protected health information (PHI) of 4,765 individuals. The PHI involved included names, dates of birth, Social Security numbers, claims information, diagnoses/conditions, medications, and other treatment and financial information. The CE strengthened its administrative, technical, and security safeguards in response to the breach. OCR provided the CE with technical assistance.
