OCR description
Riverdale Mental Health dba Mosaic Mental Health, the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 7,281 individuals. The PHI involved included names, addresses, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data.
