OCR description
The covered entity (CE), Physio Logic Chiropractic and Physical Therapy, PLLC, reported that it experienced a breach incident that affected the protected health information (PHI) of 9,580 individuals. The PHI involved included names, other identifiers, medications, and treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE issued media, substitute, and individual notices and implemented additional administrative, technical, and security safeguards.
