OCR description
The covered entity (CE), Physio Logic Medicine, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 1,280 individuals. The PHI involved included names, dates of birth, home addresses, drivers’ license or state identification numbers, medical diagnoses and treatment information, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach the CE implemented additional technical and security safeguards to better protect sensitive data.
