Pacific Clear Vision Institute

Pacific Clear Vision Institute, the covered entity (CE), reported that an employee impermissibly forwarded the protected health information (PHI) of 626 individuals to her personal email account. The PHI involved included names, addresses, dates of birth, health insurance information, diagnoses, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE sanctioned the employee responsible and provided complimentary credit monitoring services.
