OCR description
The covered entity (CE), Helping the Aging Needy and Disabled, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 628 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers and diagnoses/conditions. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
