OCR description
Napa Integrated Medicine, the covered entity (CE), reported that it experienced a burglary of its office in which computers and file servers that contained the protected health information (PHI) of 600 individuals, were stolen. The PHI involved included names, dates of birth, and x-rays. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards.
