OCR description
Radius Global Solutions, the business associate (BA), reported that it experienced a malware attack that compromised the protected health information (PHI) of 135,472 individuals. The PHI involved included names, addresses, Social Security numbers, and birthdates. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the BA strengthened its administrative and technical safeguards and provided complimentary credit monitoring services.
