OCR description
The City of Philadelphia, the covered entity (CE), reported that multiple employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 6,107 individuals. The PHI involved included names, dates of birth, addresses, clinical and financial information, and other identifiers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented an enhanced alert notification to improve email monitoring detection.
