OCR description
The business associate (BA), LCS Financial Services, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 768 individuals. The PHI involved included names, addresses, and Social Security numbers. The BA and CE notified HHS and the affected individuals. In response to the breach, the BA implemented additional administrative, technical, and security safeguards.
