OCR description
WACOSA, the covered entity (CE), reported that it experienced a hacking incident that affected the protected health information (PHI) of 1,380 individuals. The breached PHI included names, addresses, dates of birth, Social Security numbers, financial information, diagnoses, and conditions. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect sensitive data. OCR also provided technical assistance.
