South Austin Health Imaging LLC dba Longhorn Imaging Center, the covered entity (CE) reported that its third-party provider experienced a ransomware incident which affected the protected health information (PHI) of 100,645 individuals. The PHI involved included names, dates of birth, medical record numbers, gender, insurance information, diagnoses, CPT codes, and provider names. The CE notified HHS, affected individuals, and the media. In response to the breach and OCR’s investigation, the CE revised its policies and procedures, implemented additional technical safeguards and trained its third-party provider.
