OCR description
Wyndemere Senior Care, the covered entity (CE), reported that it experienced a hacking attack that affected the protected health information (PHI) of 6,832 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified individuals, the media, and HHS. The CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
