OCR description
The covered entity (CE), Prestige Care, reported that it was the subject of a hacking attack that affected the protected health information (PHI) of 116,956 individuals. The PHI involved included names, dates of birth, claims information, diagnoses, and other treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Privacy and Security Rules.
