OCR description
Warren General Hospital, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 168,921 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial information, diagnoses, medications, and lab results. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE enhanced its use of existing multi-factor authentication (MFA) for all external user access to improve security. Implemented additional technical safeguards.
