Community Health Network, Inc.

Community Health Network, the covered entity (CE), reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 20,777 individuals. The breached PHI included names, addresses, dates of birth, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect sensitive data. OCR also provided technical assistance regarding the HIPAA Rules.
