OCR description
The business associate (BA), TGI Direct, reported that a software application exposed the protected health information (PHI) of 16,113 individuals. The PHI involved included names, dates of birth, addresses, diagnoses, and health insurance information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect PHI.
