OCR description
The covered entity (CE), U.S. Drug Mart, reported that its business associate (BA) experienced a cybersecurity attack which potentially affected the protected health information (PHI) of approximately 48,261 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, and medications. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
