OCR description
Oak Street Health, the covered entity (CE), reported that an employee of its vendor mailed the protected health information (PHI) of 3,169 individuals to the wrong recipients. The PHI involved included names, birthdates, addresses, phone numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE strengthened its administrative safeguards and ended its business relationship with the vendor.
