OCR description
The covered entity (CE), Neurosurgical Associates of New Jersey, reported that multiple employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 32,156 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial information, and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative, technical, and security safeguards.
