OCR description
The covered entity (CE), Waterford Country School, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 6,211 individuals. The PHI involved included names, addresses, dates or birth, drivers’ license information, diagnoses, lab results, medications, and financial and other treatment information. The CE notified HHS, affected individuals, the media and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional technical and security safeguards.
