OCR description
The covered entity (CE), City of Homer, reported that a flash drive containing the protected health information (PHI) of 1,412 individuals was misplaced. The PHI involved included names, dates of birth, Social Security numbers, medications, and diagnoses. The CE notified HHS, the affected individuals, the media, and posted a substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI. Staff were retrained on the requirement to protect and secure PHI.
