OCR description
The covered entity (CE), Cardiothoracic and Vascular Surgeons, reported that it experienced a ransomware incident that affected the electronic protected health information (ePHI) of 1,868 individuals. The ePHI involved included names, addresses, dates of birth, social security numbers, financial information, diagnoses, conditions, lab results, medications, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional technical safeguards.
