OCR description
The covered entity (CE), City of Hope, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 902,540 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, financial and health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards.
