OCR description
The covered entity (CE), Discount Emporium dba Drug Emporium, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 2,500 individuals. The PHI involved included names and medications. The CE notified HHS, the affected individuals, and the media. Following the breach, the CE shuttered all of its pharmacies; therefore, the investigation was closed.
