OCR description
The covered entity (CE), Mount Carmel Care Center, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 1,430 individuals. The PHI involved included clinical and financial data. The CE notified HHS, the affected individuals, and the media. In response to the breach the CE implemented additional administrative, technical, and security safeguards. Staff were retrained.
