OCR description
The business associate (BA), AccessOne Medcard, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 8,049 individuals. The PHI involved included names, dates of birth, addresses, and financial information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards to better protect its PHI.
