OCR description
The covered entity (CE), Community Memorial Healthcare, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 14,798 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, and diagnoses. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals.
