OCR description
The covered entity (CE), Primary Health & Wellness Center reported that it experienced a ransomware incident that affected the protected health information (PHI) of 4,792 individuals. The PHI involved included demographic and clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice in local and regional media outlets. In response to the breach, the CE implemented additional technical security. Staff were also retrained in its requirements to protect and secure PHI.
