OCR description
The covered entity (CE), BlueCross BlueShield of Tennessee, reported that a vendor of its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 1,676 individuals. The PHI involved included names, birthdates, claims and financial information, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media.
