OCR description
The covered entity (CE), CompleteCare Health Network, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 316,059 individuals. The PHI involved included diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
