OCR description
First Choice Dental, the covered entity (CE), reported that it experienced a ransomware attack that affected the protected health information (PHI) of approximately 1,000 individuals. The breached PHI included names, addresses, dates of birth, social security numbers, claims information, diagnoses and conditions, medications, and other treatment information. The CE notified HHS and the media. The CE took several corrective actions in response to the breach implementing additional administrative and technical safeguards. OCR provided technical assistance regarding its health information privacy compliance obligations.
