Lone Peak Physical Therapy, Inc.

The covered entity (CE), Lone Peak Physical Therapy, reported that documents containing payments and billing information were stolen during a robbery. This compromised the protected health information (PHI) of 5809 individuals. The PHI involved included names, Social Security numbers, addresses, birthdates, claims and financial information, diagnoses, lab results, and medications. The CE notified HHS and affected individuals. In its mitigation efforts, the CE implemented additional physical safeguards to better protect PHI.
